Welcome to our Practice! We want you to feel at home in our professional, friendly, and caring atmosphere. The following is important information, so that you understand the way the office works for you - the most important person in our Practice.


Please arrive 15 minutes prior to your scheduled appointment time to complete the check-in process. If you are running late, please call the office to notify your physician. Patients that arrive late to their appointments run the risk of having to reschedule.

Please always bring your current insurance card and picture identification card. You are responsible to pay for your co-payment and/or any outstanding balances with Kendall Family Medicine Clinic at the time of the visit.

When you arrive at the practice, our receptionist will ask you to review your current registration information. Please let us know if your address, telephone number, insurance or other important information has changed since your last visit.

If it’s your first time coming to our practice, you will be asked to review your demographics sheet to ensure that we have all of your most updated information.

Please call us anytime at (305) 629-9644. Although we try our best to answer all calls when they come in, there may be times that you will be asked to leave a message. If this is the case, please rest assured that we try our very best to return calls as soon as possible. During lunch time there may be more of a delay in returning your call. Please make sure that you spell your first and last name and provide us with your birthday and telephone number.

Canceling Your Appointment

If you are unable to keep your appointment, we appreciate notification of at least 24 hours before your scheduled appointment. Our office personnel will be happy to reschedule your appointment. Please call

Not Feeling Well

During regular office hours, we offer same-day appointments to accommodate our patients who are having a medical emergency. After hours, there is always a provider on call to assist you and meet your urgent health care needs.

After-hours Phone Calls

If you call when the office is closed, messages are taken by our 24-hour answering service. However, if you are having a medical emergency, please dial 911 or seek attention at your nearest emergency room.

There is always a physician on call for medically urgent, non-life-threatening issues.

Please note that requests for routine medication are not emergencies. Please call during business hours for these requests.

Communicating Your Test Results

We are committed to communicating all of your lab results in a timely fashion. Many tests may require up to 7 working days of processing time before results become available. If you do not have an appointment scheduled to follow-up, you will either receive a phone call or letter from our practice as soon as your results are available and reviewed by your provider. We ask for your patience, NORMAL results will be communicated by mail. If the test is abnormal our medical assistant or physician will contact you personally when the result becomes.

We also want to encourage you to use the HEALTH PORTAL to connect with our staff and physician. This safe and convenient online patient portal allows you to:

Prescription Refills

Please have your pharmacist sent a refill request to our provider using E-prescribe. All prescription renewals that do not require an assessment of your condition will be processed within two business days of the request. Ask your pharmacist to contact us whenever your bottle states, “NO REFILLS”.

If you need to be seen by your provider or have clinical testing prior to the prescription renewal, please call our office and we will gladly make you an appointment.

Medical Record Requests

If you would like to request medical records, we ask that you fill out and sign a Medical Release Form. We use an outside company that specializes in managing compliance and correspondence copying for medical facilities nationwide.

If the records are for personal use and not to be sent to a provider directly, there is fee to copy the requested records. The final cost of your medical records will depend on the number of pages that were copied. We abide by the copying fee schedule put forth by the state of Florida. If records are to be sent to a provider directly there is not cost.

Requesting Forms to be Completed and Signed by the Physician

If you have forms that need to be completed by your provider, we request five (5) working days to ensure that they are completed thoroughly.

After-Hours & Emergencies

If you are experiencing a medical emergency, please dial 911.

However, if you need after-hours advice for an urgent medical concern, please call .

If your question is regarding a routine office matter such as appointment scheduling, insurance, prescription refills, or billing we respectfully ask that you call back during regular office hours.

Please note that requests for routine medication are not emergencies.

Accepted Insurance

We accept many insurance plans. If you have a question about your specific plan, please call . We would be glad to look into your insurance plan specifically. You may also call the customer service on your insurance card and inquire if our physicians are participating in your plan.

Our billing is centralized in our Billing Office. If you have specific questions regarding your insurance and/or billing statement(s), please call (888) 422-7720.


Kendall Family Medicine Clinic has tried to streamline the referral process for our patients. Once we have given you a referral to a specialist for services, your specialist can order most tests without any further authorizations from your insurance company without needing to go through our office. For the few services ordered by your provider that require pre-authorization, we will provide you with the referral once authorization from your insurance company has been obtained. After your visit, please stop by our check-out area so our staff can ensure we have your correct contact information. Some insurance companies may take up to seven (7) business days to provide us with the authorization for the test. If needed you may contact our Referral Coordinator by calling (305) 629-9644.
